What would cause this and should I replace it?
I wouldn’t think the throw out bearing would do this because the ID is round?
Clutch, pressure plate and throw out bearing have about 2k on them.

This is not unusual at all; in fact it's hard to find a 40 year old bearing retainer that isn't marked up like this. That doesn't mean it's a good thing, though. Your throw out bearing can catch on the irregularity and bind. After all the trouble I've had with throwout bearings I would definitely replace it.
