Well you all suspected its bad. Valve guides are trashed, oil control & 2nd rings were NFG, and there may not be enough material arround the lifter bore to install a sleeve without causing a coolant leak. So 1st thing is to clean the block sleeve the lifter bore and pressure check the block. If that goes ok the a clean-up hone, new cam bearings, and rebuild the heads with new valve guides - again.
According to the machinist the valve stems were tapered and caused the guides to fail. All other clearances were good, thats the good part. The bad is the cost $890 + the cost of the valves. Includes honing, boil block, cam bearings, freeze-out plugs, rebuild heads, guides, remove pistons bead blast & re-install.
