Is it a Bendix or a Midland type? The one I have has a remanufactured FORD part number (sticker) with the clamp. I believe it's a Midland. Is this correct for my car? Thanks..

They did use Midlands.
And if you send it out for a rebuild, save the pushrod or the rebuilder might lose it (how do I know this?)
And, paint the whole thing with the master cylinder and lid and wire if you want it stock Concours appearing. That is how the vendor supplied it to Ford,

Is the entire assembly suppose to be black? Including the cap? Thanks

Yes, this is a common mistake made in concours. The master cylinder is attached to the power booster and then the whole assembly is painted black with the cap on the MC.
