I am wondering how the upper and lower shift boots attach for a 1970. Lookes like from the assembly manual, that there are 4 screws that go down through the upper boot, through the floor and through the lower boot. But are there nuts and washers on the bottom side of the lower boot?

I just did this last weekend on my 69. The lower boot goes through the floor from the top. Which means it bolts from the top instead of from the bottom as the manual may imply. The screws go through both boots from the top and attach to the retainer plate that is welded to the underside of car.

When I put mine together, I had the shifter off the transmission. I dropped the lower boot in from the top and temporarily put the top 4 screws in to hold it. I dropped the shifter in from the top and bolted it in place. I then removed the 4 screws for the boot, put the upper boot in place and tightened it all down.

I think I might have not routed the reverse light switch wires correctly though. I just had them come out the slit in the back. I wonder of they come out the extra slit that is on top of the transmission.
 
manyc is correct. The upper ears of the lower dust boot go thru the floor opening and the bolt goes thru both dust boots to hold them in place.
All Ford factory shifter have removable handles. This allowed service technicians to install the shifter on the trans and get it in perfect adjustment while still out of the car.
Then they stab the trans in with shifter attached and adjusted, crawl into the car and attach the handle.
This is the best way by far to install your lower dust boot and adjust your shifter.
Just bolt your bellhousing to an engine stand, and bolt your trans to it. Check the picture below. Let me know if I can help with more info.

69/70 are different than 65-68. I got mine at NPD (national parts depot). They are not cheap, $60 :: but they are a nice part.
